\Instructions for PDRA Student Release Forms

1. Print out both pages of each of the forms.

2. The students need to read and review the information presented on the forms. The instructor should ask if the student has any questions.

3. Page 1 and 2 of the student release form need to be completed by the student and instructor.

4. Page 1 is the “Mailbox Copy” and Page 2 is the “Instructor Copy”. The mailbox copy must in the mailbox before the student enters the water. The instructor copy should be placed on the dash of the instructor’s vehicle so that they can be seen the outside.
