P.D.R.A. Instructors

PDRA Instructor Committee:

Effective January 1, 2012 all Instructor Permits and Membership will be one and the same.

New instructors applying for Instructor Permit/Membership will send their application to the Instructor Committee Chairman. Renewing instructors will fill out and submit the Instructor Membership Application form and the PDRA Statement of Understanding to their PDRA club representative. Instructor Membership/Permit periods are January 1 to December 31 annually.

The Instructor Guide consists of the following items (open link to read).

(2) Specific Information for Instructors (3) New Student Guest Forms

(4) PDRA Liability Responsibility Form (For Instructors who do not maintain Personal Liability Insurance)

(5) Statement of Understanding and Waiver

(6) New Instructor Check List

To make application for 2012, all instructors need to complete the following forms:

1. Instructor Member Application (3 copies)
2. Statement of Understanding and Waiver (2 copies)

3. PDRA Liability Form (If you are covered by a Dive shop policy.)

Renewing instructors: submit all paperwork and annual fees to your PDRA club

New Instructors need to meet with a member of the Instructor Committee and, in addition to the above forms, must include the following in their application packet:

4. Copy of instructor certification card,
5. Copy of professional liability insurance naming the PDRA as an additional insured,
6. Completed New Instructor Checklist

New instructors: submit all paperwork and fees to the Instructor Committee Chair:
